NCH&C services for children, young people and their families
We provide assessment, support and training through a number of pathways for children with additional and specialist healthcare needs. Our aim is always to reduce the impact of a child’s difficulties on their health and general wellbeing and help to develop the skills of the family and the workforce around the child/young person.
NCHC services are generally available for all children aged 0-18 (0-19 if in special education) who are registered with a GP within the geographical area covered by Norwich CCG, South Norfolk CCG (excluding Thetford) and North Norfolk CCG. Access to NCHC services is via our Single Point of Referral.
The services are delivered through pathways in a range of clinic and community based settings, as close to home as possible, in order to maximise physical health, reduce inequalities and reduce social exclusion.
We work in close collaboration with lots of other providers, in order to join up the healthcare in a way which works best for families and young people.
